Proof
of Age card
To obtain a Proof of Age card the applicant must attend any
ACT Government Shopfront
or the
Civic Driver Licence Service and provide original copies of:
 Proof
of identity and residency
Proof
of age
If the applicant has an ACT Driver licence it must
be sighted as proof of identity.
The fee for a Proof of Age card is $5.00. A
replacement Proof of Age card will cost $31.00
(fees subject to change).
Fraudulent applications for a Proof of Age card
will lead to prosecution against the applicant.
