Proof
of Age card
To obtain a Proof of Age card the applicant must
be 18 years of age or older on the day of
application. The application must be made in person
at any
Canberra Connect Shopfront
or the
Civic Driver Licence Service and provide original copies of:
 Proof
of identity and residency
Proof
of age
If the applicant has an ACT Driver licence it must
be sighted as proof of identity.
The fee for a Proof of Age card is $5.00. A
replacement Proof of Age card will cost $32.00
(fees subject to change).
Fraudulent applications for a Proof of Age card
will lead to prosecution against the applicant.
